The project aims to improve the offer, coordination and outreach of community health and social services to the most vulnerable people by capacity building local authorities (planning, management, coordination) and service providers (GP, social services, community nursing, auxiliary services).

Local authorities are strengthened to conceptualise, implement and monitor local integrated care projects. This includes capacity building, Coaching and monitoring of local stakeholders and authorities. Existing services will be assessed and additional services developed where needed targeting most vulnerable populations, The implementation is followed through baseline and follow up surveys to assess effectiveness of the intervention. Based on its findings, models including activity packages and lessons learnt will be developed and communicated at the end of the project phase.


Erreichte Resultate:  

(by the end of 2016)

  • 4 pilot projects contracts have been signed in March 2016; 2 pilot project contracts have been signed in April 2016; the 7th pilot project contract to be signed in early 2017;
  • All 6 projects started implementation with project teams preparations and procurement activities;
  • Complementary services – baseline survey and coaching – have been started immediately after projects’ signatures;
  • Baseline study report has been elaborated;
  • Coaching team provides support and develops solutions to upcoming problems through periodic project sites visits;
  • First common workshop has been organised in December 2016 with focus on implementation status, reporting processes. discuss implementation barriers;

Erwartete Resultate:  

  • Local communities/clusters of communities successfully implemented pilot projects to strengthen the Integration of health and social services at their levels
  • Romanian stakeholders benefit from program expérience, évidence and lessons learnt for planning future activities and policy making Integrated local health and social protection policies and plans are elaborated
  • Service packages for vulnerable catégories are well defined at community levels Community service providers' capacity on interdisciplinary team approach is strengthen New integrated services are developed based on the population's needs
